DuPage Pediatrics serves as a Medical Home, especially for children with special health care needs. The Medical Home is a relationship between the family and our health care professionals who care for your child. As part of our Medical Home, we have formed a professional/parent team to help us promote quality care and deliver expanded services to children with special health care needs. lf you have questions about the Medical Home or if you think your child would benefit from Medical Home services, please contact us.

Division of Specialized Care for Children (DSCC)

Division of Specialized Care for Children (DSCC) is the Illinois Title V agency that provides care coordination for families and children with special health care needs. DSCC, governed by administrative rules, helps children with disabilities, and those who have conditions which may lead to disabilities, grow and develop to the full extent of their abilities.

Family Voices:

A national grassroots network of families and friends, advocates for health care services that are family centered, community, based, comprehensive, coordinated and culturally competent for all children and youth with special healthcare needs, promotes the inclusion of all families as decision makers at all levels of health care, and supports essential partnerships between families and professionals.

The Illinois Life Span

Project provides resources and advocacy for children with developmental disabilities. This includes tools to promote and support advocacy for children with developmental disabilities. This includes tools to promote and support advocacy, event calendar announcing statewide and local events focused on disability-related issues, trainings, seminar videos, surveys, online chats and more.
